the answer is a

if the 45 yo has a normal u/s, he doesn't have pckd. and pckd would not reoccur in a donated organ.

Polycystic kidney can present later than 45 years of age. It could develop later in either the donor or in the transplanted kidney. Different blood groups is not a contraindication if they are compatible (for instance O can donate to A or B) if HLA compatibility is good and the T cell cross match is negative.
