Your kidneys have many types of filtration systems within them (nephrons) which have holes that are only large enough for things like water and small waste molecules to pass through, if larger molecules such as protein or blood cells pass then it typically means there's either injury or an infection in which is allowing for larger cells to pass to try and fix the problem.
