I had that problem right before I went on dialysis. One of the nurses at my kidney doctor's recommended a lotion called Sarna, and it was great. It's a little expensive, but well worth it.
It's available at most drugstores.

I use a lotion called Uremol, you can get it at Walmart or any drugstore.
