She's going into renal failure. Proteinuria means that she is losing proteins in her urine, which are normally reabsorbed. An increased creatinine level indicates that her renal function is decreasing because creatinine is filtered through the kidneys and a lot is not normally lost in urine. Inulin is filtered in the kidneys and decreased levels of it indicate that the kidneys are not working properly because they are filtering less than the amount they should be (around 120 mL per min).

renal failure--gfr would be lowered as you have elevated blood creatinine (a substance that is neither secreted nor absorbed) and glomerular membrane barrier is allowing proteins to pass through the filtration mech. of kidney, so you'd also see a low serum albumin level in this pt. the family hx of PKD indicates that she has propensity to develop a renal problem.

Renal failure, possible polycystic kidney disease. Remember to look for intracranial aneurysms. There is an association.
