Depends whether you're talking UK salaries or USA. In UK, all consultants (the most senior doctors) are paid according to a scale

Top of the scale with no private practice (purely NHS work) for a basic hours post (40 hours) and no on call is around 100, 000 pounds-thats after working for 8 years as a consultant, and it would take around 15 years after qualifying as a doctor to get to be a consultant nephrologist.
