You probably won't qualify for social security, since you have to have paid into the system.
However, there are programs to help needy families with medical costs. Contact social services and explain your situation. If possible, have your parents help you. Social Services can tell you who to contact for help. You probably qualify for Medicaid if you're not already using it, as well.

You don't have enough history to draw disability benefits. If you are really sickly, you might apply for SSI benefits.
