Insidermedicine In Depth - November 9, 2010 - Early Dialysis May Be Dangerous

Among relatively healthy patients with advanced kidney disease, holding off on dialysis may be beneficial, according to research published online ahead of print in the Archives of Internal Medicine. Here is some information about dialysis: • It is a procedure in which patients are hooked up to a machine that filters their blood the way the kidney normally does • It is a treatment used for those with advanced kidney disease • It generally helps those with severe kidney disease live longer and have a better quality of life Researchers from William Jennings Bryan Dorn Veterans Hospital and the University of South Carolina looked at death rates among over 80000 individuals who had started dialysis for advanced kidney disease but were otherwise relatively healthy. The investigators looked at whether the point at which the patients started dialysis affected their death rates. The higher the estimated glomerular filtration rate when patients started dialysis, the more likely they were to die after one year. Estimated glomerular filtration rate is a measure of kidney functioning, with higher levels indicating a better functioning kidney. Today's research suggests that it is most beneficial to hold off on dialysis treatment until absolutely necessary among relatively healthy patients with advanced kidney disease. These findings may not translate to patients with advanced kidney disease and other health problems, such as diabetes and heart disease.
